Waiver and Release

• Taking pictures or Audio/Visual recording of the online sessions is strictly prohibited.

• I give the Sunday School at the Hanuman Mandir permission to include my child(ren) in photos used for informational or promotional purposes.

• As legal guardian of the minor who participates in Sunday School programs, I accept unto myself all responsibility and all liability for any injury or loss or damage that occurs to me and/or to the minor as a result of the minor’s participation in Sunday School programs.

• I understand that the parents need to accompany the kids with special needs and teachers at the Sunday School program are not trained to handle students requiring an IEP.

• I accept that students will be placed in classrooms based on strict age/grade-related policies. The minimum age of acceptance is 6 years old (as of September 1, 2021) without any exceptions.

• The students must adhere to all the classroom rules and behavioral expectations laid out by the teacher. Non-compliance might result in the student’s removal from the class.

• Due to student privacy requirements, no adults other than parents or legal guardians are allowed to be in a zoom meeting with students.

In consideration of your accepting my child’s participation in the above program, I hereby, for myself and my child, waive and release The Hanuman Mandir of Greater Chicago, and the teachers/ volunteers participating in the program, from all liability or claims arising from any injury to myself, my child or my property.
